20 month female patient was vaccinated with IPV, 0.5ml using appropriate25G 5/8" needle for subcutaneous injection. Patient was vaccinated at appropriate angle and dosage. Patient received vaccine at incorrect site of inner right thigh. Parent reports no adverse reaction following IPV.
