the syringe failed and some of the medication was not administered; Possible vaccine exposure; This non-serious case was reported by a other health professional via call center representative and described the occurrence of incomplete dose administered in a 63-year-old patient who received Herpes zoster (Shingrix) (batch number C9X3R, expiry date 05-JUN-2027) for prophylaxis. On 16-NOV-2025, the patient received the 2nd dose of Shingrix (intramuscular). On 16-NOV-2025, an unknown time after receiving Shingrix, the patient experienced incomplete dose administered (Verbatim: the syringe failed and some of the medication was not administered) and exposure via skin contact (Verbatim: Possible vaccine exposure). The outcome of the incomplete dose administered and exposure via skin contact were not applicable. Additional Information: GSK Receipt date: 18-NOV-2025 A Certified Pharmacy Technician called on behalf of a pharmacist to report an administration incident involving Shingrix. While the pharmacist was administering the second dose, the syringe failed and some of the vaccine leaked out as the syringe was being pushed into the patient. The full intended dose was therefore not delivered. The reporter believes the patient was male but could not confirm gender or provide identifying details.
