Symptom narrative

redness and rash at injection site noted 15 minutes after injection, provider notified and pt was observed 15 more minutes, redness and rash increased and spread up onto chest. Liquid children's Benadryl was ordered and administered orally, pt was monitored for additional 30 min after meds, redness and rash has decreased and pt was released with parents.
