Symptom narrative

Incorrect preparation; only the liquid part of the vaccine was administered; This non-serious case was reported by a physician via call center representative and described the occurrence of inappropriate preparation of medication in a 14-year-old female patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723-B, expiry date 30-MAR-2028) for prophylaxis. On 21-NOV-2025, the patient received the 1st dose of Penmenvy. On 21-NOV-2025, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Incorrect preparation) and inappropriate dose of vaccine administered (Verbatim: only the liquid part of the vaccine was administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 21-NOV-2025 The doctor just administered the Penmenvy meningococcal vaccine for first time and he/she did not realized that, did not see that it needs to be reconstituted and not realized that it was different than the others meningococcal vaccines. Anyway, the doctor just administered without reconstitution and wondering what the risk was with that and kind of want some information before call the family.
