Symptom narrative

She gave a child the VAXELIS when the patient was supposed to receive Pediarix, the one without HIB component; Vaxelis administered to a 5 year-old child; This non-serious spontaneous Safety Report was received by partner, on 10-NOV-2025 and forwarded to agency on 12-NOV-2025 from a medical assistant and concerned a patient of unknown ethnicity. Due to the nature of the events, the causality between the events "She gave a child the VAXELIS when the patient was supposed to receive Pediarix, the one without HIB component" and "Vaxelis administered to a 5 year-old child" and Vaxelis was not applicable. Further information is expected.
