Patient received vaccines around 5:10pm. Immediately after receiving vaccines, pt jumped off the exam table. Felt lightheaded and had brief syncopal episode <3 seconds. NP went to room to check on pt. Pt alert, oriented and drinking water. About 2 minutes later, pt had another episode where he spit out his water and started shaking. Pt was placed on the ground by NP. Head was protected. Pt came to after less than 5 seconds. Normal breathing and skin color during episode. BP immediately after 140/84. Pt laid on the ground for 15 minutes. Felt fine but tired. No difficulty breathing. Pt stayed in office for about 45 minutes after vaccine. No other events. Pt alert and oriented. Exam normal. BP normalized. HR, RR and SpO2 normal. Exam normal.
