Symptom narrative

Inbound call from nurse who reports expired PROQUAD was administered to one patient. Product had expiry date of 10/30/2025 and was administered 11/5/2025. Product did not experience any temperature excursions through the date of administration. No ad; No additional AE; This spontaneous report was received from a Nurse and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 05-Nov-2025, the patient was vaccinated with an expired dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Y008785, expiration date: 30-Oct-2025); diluted with a sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported) (anatomical location and administration route were not reported) as prophylaxis. No additional adverse events were reported.
