When doing last Reconciliation I identified a Pentecel was given incorrectly. The green top powder was given but not mixed with the blue top liquid that remains in the box. I contacted the manufacture and the provider. I was informed that it should not be harmful to the children since only part of the vaccine was given. The employee could not remember what liquid was given but though it had to be a sterile Diluent. We will need to give this vaccine again as the last one is invalid. I called and informed the guardian who verbalized understanding. I submitted a VEARS and internal Incident Report, reporting this situation and have had serious talks with staff.
