Symptom narrative

> Immediately after receiving her vaccines, the patient became unresponsive for approximately 2-4 seconds while sitting at her father's lap. Parents reported no previous history of fainting after vaccinations. > RN observed a brief syncope/vasovagal episode. RN stimulated with verbal cues by calling her name and gently tapping to arms. Patient responded immediately after 2 seconds. Vital signs taken by RN are as follows : BP : 85/65; temperature : 97.2 F; PR : 65bpm. No injury noted. No report of falling from parents. Sugary drink (apple juice) provided. RN elevated child's lower extremities. RN observed for 15 min post-event. > Vasovagal episode following immunization. Patient stable post IZ. > Child was responsive. Child started ambulating appropriately. Child verbalized feeling well. Patient discharged with parents in stable condition after 15-minute observation.

None as per stated in the health screening questionnaire.

Other medications

Mom stated she gave 5mls of Tylenol to the patient prior visit to Immunization clinic despite patient was feeling fine prior to immunization.

Allergies

Patient's mother stated and wrote unsure in the health screening questionnaire pertaining to history of allergy.
