Symptom narrative

Patient called pharmacy to get a copy of VAR form filled out by patient prior vaccine. When asked why, he informed RPH that he was sure he had stated that he had an egg allergy on the VAR and after receiving vaccine he had an allergic reaction and had to go to the emergency room.
