Symptom narrative

Patient was intended to receive DTaP and Hib, but was mistakenly administered the flu vaccine. Error discovered when I asked to see the vial from the MA and she told me it was the HIB vaccine, I asked to see the vial again and she told me she gave the flu vaccine on accident. No adverse reaction noted at this time. Reported to prevent recurrence and for public health tracking of vaccine administration errors.

Ear infection on 11/20, was on 10 day antibiotic course
