Symptom narrative

Patient reported to the clinic on 11/20/25 to see PA due to reports of a rash- red spots all over body. Provider determined it was likely pityriasis rosea 2/2 Prevnar 21 vaccine. Provided education and reassurance for patient. Can try topical steroid 2-3 times per day as needed for itching. Discussed that oral steroids do not cause quicker resolution of the rash and suggest avoiding if possible. Discussed rash may persist for two to three months. Because pityriasis can mimic syphilis rash, will check RPR as r/o although feel this is extremely unlikely with her sexual history. Encouraged follow-up with PCP in 2-3 weeks for recheck. Patient should be seen sooner if the rash worsens or for new symptoms. All questions answered. Prescribed Triderm 0.1% topical cream apply a thin layer to affected area(s) 2-3 times per day for up to 2 weeks.
