On 05-Nov-2025, the patient was inadvertently concomitantly vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX REFRIGERATOR STABLE), Solution for injection (lot #Y017915, expiration date: 28-Oct-2026) and Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), Solution for injection (lot #Z011859, expiration date: 13-Jan-2027) instead of Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine (M-M-R II) (accidental overdose; wrong vaccine administered).
