Inappropriate Reconstitution Technique for Menveo (2-Vial) vaccine; Inappropriate Reconstitution Technique for Menveo (2-Vial) vaccine; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 21-year-old female patient who received Men ACWY-CRM NVS (Menveo) for prophylaxis. On 25-NOV-2025, the patient received Menveo. On 25-NOV-2025, an unknown time after receiving Menveo, the patient experienced inappropriate preparation of medication (Verbatim: Inappropriate Reconstitution Technique for Menveo (2-Vial) vaccine) and inappropriate dose of vaccine administered (Verbatim: Inappropriate Reconstitution Technique for Menveo (2-Vial) vaccine). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date: 25-NOV-2025 On 25-Nov-2025, a medical assistant reported that the Menveo (2 vial) vaccine had been administered to a patient using a non GSK supplied diluent (live vaccine sterile water diluent). The batch number was not provided and a request for this information has been made.
