Hiberix administration of diluent alone without powder; Hiberix administration of diluent alone without powder; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 13-month-old female patient who received Hib (Hiberix) (batch number 742G5, expiry date 26-AUG-2026) for prophylaxis. On 29-OCT-2025, the patient received the 1st dose of Hiberix. On 29-OCT-2025, an unknown time after receiving Hiberix, the patient experienced inappropriate preparation of medication (Verbatim: Hiberix administration of diluent alone without powder) and inappropriate dose of vaccine administered (Verbatim: Hiberix administration of diluent alone without powder). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date: 25-NOV-2025 On 29-Oct-2025, a nurse reported that a female child had received Hiberix sterile water alone without the powder with lot 742G5 and expiry on 26-Aug-2026. The nurse stated that they were going to administer a full appropriate dose afterwards.
