Reconstitution/Administration Errors; Reconstitution/Administration Errors; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 18-year-old male patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723, expiry date 08-OCT-2026) for prophylaxis. On 24-NOV-2025, the patient received the 1st dose of Penmenvy. On 24-NOV-2025, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Reconstitution/Administration Errors) and inappropriate dose of vaccine administered (Verbatim: Reconstitution/Administration Errors). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 25-NOV-2025 On 25-Nov-2025, the reporter stated that a patient was inadvertently administered only the MenB portion of PENMENVY. The reporter asked whether revaccination was required and what steps should be taken following the incident. The batch number was not provided and a request for this information has been made.
