Symptom narrative

(employee) was doing the vaccine count and noticed it was off from the database from 11/26/2025. Our office was closed on 11/27/2025. Employee found that (employee) gave the patient the Saline Diluent Component .4% sodium Chloride without the active powder for the vaccine. Employee then notified FNP-C about the situation. Provider reached out to myself (office manager). Provider reached out to manufacturer. I also had the provider reach out to the patient to see if they had any reaction. The parent stated that they will bring the patient back in for the vaccine after the holidays. The parent stated that the patient had no reaction.
