Incorrect Preparation; accidentally gave only the diluent; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 50-year-old male patient who received Herpes zoster (Shingrix) for prophylaxis. On 20-NOV-2025, the patient received Shingrix. On 20-NOV-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Incorrect Preparation) and inappropriate dose of vaccine administered (Verbatim: accidentally gave only the diluent). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 21-NOV-2025 Medical assistant reported that yesterday, they had a new year nurse professional student, and he did not know that he needed to mix the Shingrix with the diluent and the powder. So, he accidentally gave only the diluent. The batch number was not provided upon follow up with the reporter.
