Symptom narrative

Patient received the vaccination on 11/25/25 and her skin started turning red and swollen the next day11/26/25 in the morning, pain not subsiding, the injection site hot, came to the phy on 11/28/25, she did state tried applying cold pack but no use, told her to go to ER if needed, but she refused, told her to take NSAIDS if needed for pain and swelling and wait for 1 more day and suggested to go to ER no matter what if symptoms does not resolve.
