Symptom narrative

Patient came in on 11/29/2025 at 1:45 PM, to inform us that she had experienced a reaction to the Shingles vaccine, Shingrix. She complained of blisters on the arms and hands. She said that she first had the reaction 30 minutes after the vaccine, she had gone to Urgent Care and Hospital on 11/05/2025 where she was told that the blisters were due to the vaccine. They had told her to use an antiseptic wash and to keep the area clean and dry. She had been following the directions from the Drs from that day after. On 11/27/2025 she had started to apply OTC triple antibiotic ointment to the blisters, she had said that the blisters on the hand and wrist had not healed due to daily use such as reaching into pockets as well as to carry groceries and other daily activities. The blisters on the forearm had started to heal. Blisters had been seen on both arms, forearm area as well as the hands. Patient was advised to go back to Urgent Care.
