RN noticed that there was no powder in the vial to be mixed.; This non-serious case was reported by a other health professional and described the occurrence of intercepted product preparation error in a patient who received Hib (Hiberix) (batch number BS59L, expiry date 24-AUG-2026) for prophylaxis. On 24-NOV-2025, the patient received Hiberix. On 24-NOV-2025, an unknown time after receiving Hiberix, the patient experienced intercepted product preparation error (Verbatim: RN noticed that there was no powder in the vial to be mixed.). The outcome of the intercepted product preparation error was not applicable. Additional Information: GSK Receipt Date: 25-NOV-2025 The registered nurse went to draw up a Hib vaccine for a patient. When the nurse attempted to add the diluent into the vial, she noticed that there was no powder present to be mixed. As a result, the vaccine was not administered. The batch number was not provided and a request for this information has been made.
