Underdose

Symptom narrative

Underdose (Half dose administered to a patient); This non-serious case was reported by a pharmacist via call center representative and described the occurrence of underdose in a 55-year-old female patient who received Herpes zoster (Shingrix) (batch number ZB7DJ, expiry date 16-JUN-2027) for prophylaxis. On 21-NOV-2025, the patient received the 2nd dose of Shingrix. On 21-NOV-2025, an unknown time after receiving Shingrix, the patient experienced underdose (Verbatim: Underdose (Half dose administered to a patient)). The outcome of the underdose was not applicable. Additional Information: GSK receipt date: 21-NOV-2025 The registered pharmacist called to report that a patient was administered half the dose (less than 0.45 milliliters) of the Shingrix vaccine. No further information was obtained. The reporter consented to follow up.
