Symptom narrative

14mons old patient on a 'modified' vaccine schedule received extra dose of PCV20 and Hib. She had received both a month earlier (09/2025). However, she was pre-scheduled to receive both on the administered date (10/2025). The prescheduled date was not cancelled, and pt got the same set again. No adverse event was identified. Parent was notified of error. Patient was monitored by parent, office stayed in touch, and no identified adverse event was reported by parents.
