Symptom narrative

Insert my name on date of presentation, approximately 32 days after vaccination, patient still with 20 cm x 5 cm ill-defined erythema, pain on right flexural surface of forearm. Lymphadenopathy not appreciable at time of examination. Patient states worsening headaches. Occasional nerve pain on right side. Also administered PCV Pfizer, lot LN 4930 at time of vaccination.
