Reported Symptoms: 10039906:SEIZURE; Narrative: pt found laying on the floor, they suspect he fell, pt having possible seizure activity noted. Staff assisted pt to a chair, pt non- responsive to verbal, called me. I contacted someone to assist w/ transfer of pt. I arrived, pt in chair, non-responsive to command. Right pupil lateral, left pupil midline, sluggish at this time. Pt not following commands, right hand tremor/twitch present. Pt had voided on himself when found on the floor per staff. Pt's provider has not been contacted, I instructed staff to do that. I contacted the on-call provider, informed them of pt's status and the need for a head CT. They entered orders for get CT stat. Pt loaded on stretcher, staff and police present to assist w/ transfer of pt to another bldg. Pt on O2 and monitor for transfer. 0554: Pt en route to CT. Pt is now able to say a few words and answer single word questions when asked. Pt answered "yes" to questions of pain or headache. Pt states the year is 1969. 0559: pt in CT, awaiting orders for head CT to be done. Pt laying flat at this time, no tremor noted, P 59, noted. 0605: Non-contrast head CT completed at this time. 0614: Pt back on the gurney, transferred to MICU. Pt more responsive to verbal command. Following some instructions when asked. Denies pain at this time. 0618: Pt in MICU, monitoring equipment applied, pt continues to be more responsive and following commands. Other Relevant HX: Other:
