Symptom narrative

Reported Symptoms: 10002391:ANGINAL PAIN; 10013781:DRY MOUTH; 10020772:HYPERTENSION; 10043071:TACHYCARDIA; Narrative: Other Relevant HX: 63 y/o F patient with c/o reaction status post 2nd moderna vaccination. Pt. is a/o x4. Facility called police dispatch of a patient that needed transport to ED. Upon arrival report was given by staff where he stated that the pt. had just received her second vaccination and then became hypertensive, shakey, and shortness of breath. Staff had given 25mg of dyphenhydramine via oral solution. Pt. states that the first vaccination wasn't bad at all and she didn't think that this one would be either. Pt. states that she started to have a major headache on the way to the ED while in the ambulance. Pt. states that while in route that the chest tightness was resolving but the headache was coming on pretty strongly. Pt. transported to the ED without incident Other: DRY MOUTH
