Symptom narrative

Systemic: Allergic: Rash (specify: facial area, extremeties)-Medium, Systemic: stinging-Medium, Additional Details: Patient returned to clinic 11/21/2025 for scheduled f/up appointment for 2nd dose of Heplisav-b vaccine. Patient stated he had a reaction to vaccine 7 hrs after receiving Heplisav-b in clinic. States started with rash (multiple small bumps) on right arm, rash burning. Then felt stings to right arm, and then to different areas across his body. Reports Rash subsided with in 24-48 hrs, stinging slowly improved w/in 7 days. Denies SOB, swelling, no nausea, no abdominal pain.
