Symptom narrative

Site: Redness at Injection Site-Mild, Additional Details: On 11/25/25 the patient came to get 2nd shot of shingrix. The patient was given a shingrix vaccine which was mixed more than 4 hours ago. On 11/26/25 the pharmacist discovered this incidence and reached out to patient's primary provider. Per MD's instruction, the patient should receive a repeated dose as the previous dose may not be as effective as it possible to be. The pharmacist contacted patient and scheduled a following dose. The patient reported no adverse events.
