Symptom narrative

MMR and Proquad were given erroneously at the same visit. Varicella (Varivax) should have been given instead of the Proquad. Mother of the child was made aware of the mistake and was told to watch for a fever and how to properly treat. Incident was charted in patients' chart and follow up was completed.
