The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 05-Nov-2025, the patient was inadvertently administered a dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), Solution for injection, lot #Y016874, expiration date: 07-Apr-2026, dose number 3, 0.5 mL (reported as 0.5 mL / Two Dose Series) administered by Intramuscular route for prophylaxis (extra dose administered, product administered to patient of inappropriate age). The suspect vaccina was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT) administered by Intramuscular route (indication, expiration date, and lot # were not reported). It was stated that the patient received a dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD( inadvertently), due to low titers on that day (also reported as yesterday) that required revaccination from direction from the school that she was attending (antibody test negative). The nurse confirmed the patient has not reported any medical concerns regarding this report. She also stated that the patient was new to their practice and they did not have any additional information to provide. At the reporting time, the outcome of low titers was not provided. The action taken with the suspect vaccine and sterile diluent was not applicable.
