Symptom narrative

Patient reports blood clots in legs/lung as well as bad cough/difficulty breathing after last covid vaccination. Patient states this led to an emergency room visit followed by hospitalization due to stroke. Patient states she was placed on a ventilator for 11 days and she delt with bed sores that were ongoing until December 2024.
