Symptom narrative

Resident called this am and requested an RSV vaccine as she could not participate in the mobile clinic. The home did a mobile clinic on 11/20/2025 to provide the residents with RSV, PCV20 vaccines. This resident came to the mobile clinic and said she couldn?t stay as she had to go to an appointment. Residents? name was added to the list of residents that we would have to go back and vaccinate at a later date. This nurse took the call this morning 12/2/2025 and remembered this resident coming to the mobile clinic. This nurse told resident she would go to home today and give her the RSV vaccine. This nurse did not look up patients record in system before going and administering the vaccine. 1100 Resident completed the assessment form and vaccine administered without difficulty. This nurse returned to the home to add the vaccine to system and noted the resident already had 1 dose Abrysvo RSV vaccine administered in 2023. The current recommendations for RSV vaccine are only 1 dose, and it is not an annual vaccine. This nurse reported this to the director. This nurse called and spoke with patient and explained that she had already had the vaccine and didn?t need the 2nd dose. This nurse explained that current recommendations had changed for 2023. Resident voiced understanding and denies any ill effects from vaccine. Report of the event completed.
