Symptom narrative

The patient was given an HD flu shot 11/03/2025. A few days later, the husband of the patient came into the pharmacy and said the patient went to the hospital that same evening for chest issues. We were notified by the family that she had passed on Nov 17th.

HYPERTENSION, HYPERLIPIDEMIA, ANGINA PECTORIS(I25.118)
