Symptom narrative

Patient came to pharmacy to receive Covid Novavax vaccine (a protein based adjuvanted vaccine). She was given Covid Moderna Spikevax (a mRNA vaccine) instead. Patient's mother was informed right after the incident. We apologized and emphasized the vaccine given safe and effective, and patient had received 4 Covid Pfizer (also mRNA) vaccines in the past according to state database. After consultation, the mother was understanding and accepted our apology.
