Symptom narrative

Patient reports that approx 4 months after recieving vaccines he experienced tinnitus and hearing loss. Patient also gradually lost sense of taste and periodically loses sense of smell. Patient reports that these symtoms have not resolved. Patient has seen an audiologist who cofirmed hearing loss and initiated hearing aids.
