Symptom narrative

After receiving the flu shot, patient stood to continue in store shopping and stated she felt dizzy and was having trouble breathing. Patient checked no to all pertinent information on consent form. Upon further questioning, patient admitted that she had been sick for the past few days, was nonadherent to her COPD medications, and was experiencing blurred vision on the drive over to receive vaccination. We called 911 and upon EMS arrival, patient refused care. Checked in with patient after about 10-15 minutes and she stated she was feeling fine with no issues.
