Symptom narrative

Patient first called in state swelling in his arm around injection site which is tender (roughly 3 inch diameter), he noticed the swelling on 12/03/25 at 9am (48 hours after vaccine administration). He did not notice swelling prior. After asking him a series of question he did mention feeling "flu like symptoms at night the same day of vaccine 12/1/25 but the symptoms resolved on 12/2/25 around noon. The injection site is still swollen and tender and patient will keep an eye on it.
