Symptom narrative

Staff member inadvertently pulled HPV vaccine instead of HIB vaccine that were in the fridge on the shelf under neath each other. Vaccines were ordered in the computer as HIB vaccine instead of HPV vaccine and then administered incorrect vaccine due to pulling the wrong vaccine .
