Patient was given incorrect vaccine. Mother requested a proquad vaccine not a kinrix. Both vaccines were drawn up and ready to administer but mother changed her mind before vaccinating and wrong vaccine was grabbed to be administered. Incorrect vaccine was not found until the next day when inventory was completed and the nurse was notified. The nurse contacted mom and notified her of the error. Mom had called previous to finding the error stating that patient had developed a fever of up to 105, c/o leg pain and difficulty walking.
