Symptom narrative

After discussing with patient, he stated that a day after the vaccination he began to feel pain at the injection site of right arm. Later that day, swelling of the arm occurred. He stated that the pain in the arm lasted roughly about 6 months. over the counter pain reliefer was used and the side effect eventully went away. Patient consulted with MD about booster but was not recommended by Primary Care Provider. CSP
