Pt received influenza vaccine 11/17/25, presented to [name withheld] ED in [city, state withheld] on 11/29/2025 regarding bilateral lower extremity weakness to the point where he couldn't walk on 11/28 that rapidly progressed to upper extremities 11/29. Guillain-Barré syndrome.

Current illness

1. Wound cellulitis 2. Type 2 diabetes mellitus with foot ulcer 3. Non-pressure chronic ulcer of left heel and midfoot limited to breakdown of skin

Type 2 diabetes mellitus, Obestiy, Hypertension, Hyperlipidemia, Sjogrens Syndrome, CKD stage III, osteoarthritis, cushings syndrome, sleep apnea
