Symptom narrative

Patient reported left arm twitched upwards during vaccine administration. One day later, patient experienced intermittent mild soreness & intermittent mild burning sensation on left lateral shoulder area & intermittent mild numbness & tingling radiating to 1st & 2nd left fingers Soreness & burning do not radiate. 2-3 days after receiving vaccine, patient noticed bruise on left lateral shoulder area. On exam 12/3/25, patient with 2x3 cm ecchymosis with tenderness to palpation on left lateral shoulder area; full range of motion of neck, bilateral shoulders, wrists, hands, & fingesr; fingers sensation intact; 5/5 strength of bilateral shoulders, wrists, hands, & fingers.Recommended treatment includes ice to L shoulder area, avoidance of upper body exercises & heavy lifting, & follow up visit in 1 week.
