Provider ordered Dtap at office visit on 12/2/25. This would be patient's 4th Dtap vaccine. Medical assistant gave patient Tdap instead. 12/4/25, RN reviewed vaccine consents from 12/2/25 and noticed the mistake and informed provider and vaccine coordinator. This RN called patient's parent and spoke to patient's father. This RN explained patient was given Tdap instead of Dtap at office visit. Patient's father denied patient having any adverse reaction such as rash, difficulty breathing, swelling of the face or throat, hives, fever, or lethargy. Patient' dad reports patient is doing fine. This RN informed patient's dad, per CDC recommendations, "Tdap given to a child younger than 7 years as either dose 4 or 5 can be counted as valid for Dtap dose 4 or 5." Patient's dad verbalized understanding.
