Symptom narrative

Patient received annual flu vaccine in left deltoid on 25 Nov 2025 around 1147 local time. Later that evening patient reported redness, tenderness, pain, warm to touch, and increased itchiness to local area the size of a quarter. The next day on 26 Nov 2025 patient noticed vaccine area has doubled in size similar to a tennis ball. MOP contacted nurse advice line and PCM, advised to treat at home with Benadryl and cortisone cream, But to seek care if symptoms worsened. 27 Nov 2025 the redness, tenderness, pain, warm to touch, and increased itchiness to his left shoulder extending down to his left elbow. MOP MOP contacted nurse advice line and PCM, advised to treat at home with Benadryl and cortisone cream, But to seek care if symptoms worsened. 28 Nov 2025 area has decreased in size some but still reports redness, tenderness, pain, warm to touch, and increased itchiness. 01 Dec 2025 patient is now symptom free
