Symptom narrative

No adverse event; approximately 70% of a PROQUAD dose; This spontaneous report was received from a Nurse and refers to a 4-year-old female patient. The patient's pertinent medical history, concurrent conditions, concomitant therapy, drug Reactions and allergies were not reported. On 11-Nov-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (lot #Z008867, expiration date: 10-Nov-2026) 0.5 mL for Prophylaxis. The vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (lot # and expiration date was not reported). The reporter reported that the patient moved during PROQUAD administration and received approximately 70% of a PROQUAD dose (Underdose). No symptoms reported. No additional AE.
