Symptom narrative

Pt received 3rd HPV dose in Left Deltoid and Influenza Vaccine in Right Deltoid. Influenza vaccine was the last vaccine to be administered. After administration of flu vaccine, pt had slight bleeding localized to vaccine site after removal of needle. Pt was in an upright sitting position on the exam room bed. Pt visualized blood and began to slowly fall toward her left side. Pt hit frontal head on computer station in exam room during fall, and then gradually sank to floor. Entire fall lasted about 3-5 seconds. Pt appeared to not be alert for 5-10 seconds. This MA notified team RN. Medical emergency was initiated.
