Received one vaccine today, the 720 ELU per .5 ml; Received one vaccine today, the 720 ELU per .5 ml; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate dose of vaccine administered in a 59-year-old male patient who received HAV (Havrix 720) (batch number 42DM9, expiry date 22-JAN-2027) for prophylaxis. On 02-DEC-2025, the patient received the 1st dose of Havrix 720 .5 ml. On 02-DEC-2025, an unknown time after receiving Havrix 720, the patient experienced inappropriate dose of vaccine administered (Verbatim: Received one vaccine today, the 720 ELU per .5 ml) and underdose (Verbatim: Received one vaccine today, the 720 ELU per .5 ml). The outcome of the inappropriate dose of vaccine administered and underdose were not applicable. Additional Information: GSK receipt date: 02-DEC-2025 Nurse practitioner had a question about the hepatitis A vaccine, the Havrix, for dosing. The reporter's specific question was for the patient to get the full milligram the full adult dosing; they need to space to the part with the junior dosing, or all they had available there. The reporter asked that they need to give the second one right now or have to space it apart.
