Symptom narrative

Patient presented to Clinic on 12/03/25 for routine vaccinations for school. Child's father accompanying the patient during the visit. Child and father then presented to clinic on 12/05/25 due to child having increased swelling and redness to the top of his left arm, by his shoulder. Child's shoulder is visibly red, swollen, and hot to the touch. Patient's skin is "tight" feeling per the patient where his arm is swollen. Patient and father deny any fever since the vaccines were given.
