PATIENT WAS ADMINISTERED 1ST DOSE OF MMRII VACCINE ON 11/7/2025. SHE STATED ON HER VARS THAT SHE WAS NOT PREGNANT OR PLANNING TO BECOME PREGNANT. SHE PRESENTED TO PHARMACY ON 12/5/2025 FOR HER SECOND DOSE AND TOLD US SHE WAS PREGNANT . SHE TOLD US SHE WAS PREGNANT FOR HER 1ST DOSE AS WELL. WE SUBSEQUENTLY EXPLAINED TO HER THAT WE DID NOT KNOW SHE WAS PREGNANT AND THAT WE CANNOT VACCINATE HER AND REFERRED HER TO HER PHYSICIAN.
