Symptom narrative

Patient received vaccine as part of our annual flu vaccine clinic. Vaccine was administered by another nurse who at the time was employee by the facility who volunteered for the clinic. Vaccine was received on 10/14/2025. On 10/16/2025 patient approached me (employee health nurse) and showed me her arm where the vaccine had been administered,. It was red and swollen. I advised her to see her PCP. She saw her PCP on 10/18/2025. Per patient the provider stated that the vaccine had been administered "too low" on her arm and that the vaccine went into tissue, not into muscle. Per patient this was verified by x-ray. Patient saw her PCP again several days later and was diagnosed with calcific tendinitis. States that she has trouble lifting her arm. Was advised to see an orthopedic surgeon. She did and the orthopedic surgeon told her that her pain was related to the vaccine administration.
